I managed to do them handheld, without a tripod or a monopod. For that I configured my Nikon D2H to take only 5 pictures on CL mode, at 5fps. Combined with the autobracketing mode at 5 intervals of 1 EV that means that to generate the picture above, I just need to keep the shutter button depressed and it will take only the five pictures needed in (optimaly) less than one second.
Photomatix is then used to align the pictures (granted the alignement difference is very slight due to the speed of the camera), generate the HDR picture and do the tone mapping.
